Team Manager Home Care (Registered Nurse)

About the role

The Team Manager leads the Patient Care Team, ensuring continuity of care from admission to discharge or transfer to bereavement. They oversee all patient care services, including volunteer and bereavement, and collaborate with healthcare providers to manage palliative care measures.

Responsibilities

• Supervise, evaluate, and coordinate interdisciplinary team members
• Affirm continuity of care from admission to discharge or transfer to bereavement
• Serve as patient advocate and coordinator for social service and health care providers
• Maintain and document patient records accurately
• Review and approve payroll, ensuring accurate time, mileage, and expenses
• Monitor and control resources to ensure cost-effective delivery of services
• Manage team schedule and provide interdisciplinary input into plan of care
• Facilitate team meetings and participate in quality improvement activities
• Assure staff competence and performance through field visits and evaluations
• Oversee staff and volunteer schedules and address customer service issues
• Perform utilization reviews and participate in professional development

Requirements

• Reliable transportation with appropriate license and insurance
• Private telephone in home
• Knowledge of primary medical and nursing care principles
• Understanding of interdisciplinary health care team dynamics
• Working knowledge of VITAS information system and management reports
• Two years supervisory experience in a health care organization

Qualifications

• Current and valid R.N. License
• Bachelor's degree preferred
