Team Lead Critical Time Intervention Program
Job Description
AHRC NYC is seeking a Team Lead for their specialty Critical Time Intervention (CTI) Program. CTI is a highly focused, structured, time-limited intensive case management program that assists clients during critical transition periods from hospital to community-based care. This role leads the delivery of Critical Time Intervention (CTI) services by building strong connections to community-based supports, addressing barriers to stability, and promoting long-term engagement, recovery, and social integration. The CTI Team Lead provides day-to-day clinical and programmatic leadership, monitors participant progress, and ensures timely problem-solving when challenges arise. They facilitate weekly team meetings, support staff development, and ensure fidelity to CTI model standards.
