Team Coordinator Weekends Only

A Part-Time Team Coordinator position at our Lexington Community Hospice House, working weekends only (Saturday and Sunday, 7am-7pm).

About the role

As a Team Coordinator, you will lead and serve by ensuring the smooth operation of the agency so patients receive the care they need. Responsibilities include answering incoming calls, processing daily workflow, assigning clinician visits, preparing interdisciplinary meetings, and managing office inventory and medical supplies. You will also support clinical leaders and have access to a supportive leadership team and professional development opportunities.

• Manage and maintain office inventory and medical supplies
• Support clinical leader(s)

Qualifications

• A heart to serve patients and families, with a passion for providing the best possible care
• Associate degree preferred
• Minimum of 1-year experience in a healthcare setting
• Scheduling and office manager experience preferred
• Reliable transportation
• Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs
