Responsibilities and Duties

• Provide managed care, support, training, and interventions to assist individuals with acquiring new skills in daily living, problem solving, communication, and social skills.
• Responsible for creating and implementing individualized service plans to assist with achieving identified goals.
• Maintain accurate and timely progress notes and additional documentation.
• Clock scheduling of regular ongoing appointments.
• Attend interdisciplinary team meetings.

Qualifications and Skills

• Bachelor’s degree in a Human Services field or accredited School/College
• Knowledge of TBI and PTSD
• Qualified Mental Health Professional (QMHP) or QMHP-T
• Valid driver’s license with acceptable DMV driving record
