Targeted Case Manager - Lewiston

About the role

The Targeted Case Manager functions at the initial entry point of the care continuum within the Certified Community Behavioral Health Center (CCBHC) by providing outreach, case management, and information about the health care system. This role supports care transitions between health care agencies (e.g., emergency department/inpatient unit to community services) and provides ongoing health education and health promotion in partnership with the Interdisciplinary Team. Integrated behavioral health care involves the systematic coordination of physical and behavioral health care.

Responsibilities

• Conduct assessments of participants with their guardian and/or natural supports for physical health, mental health, and social service needs.
• Provide information on the importance, availability, and appropriate use of primary and emergency care.
• Assist in exploring less restrictive alternatives to hospitalization.
• Coordinate and monitor the implementation of the Individual Treatment Plan (ITP) to ensure it effectively addresses the needs of the person.
• Determine appropriate level of care using evidence-based screening tools.
• Provide primary care coordination activities, including adapting supports, treatment plans, crisis plans, and/or Psychiatric Advance Directives as needed.
• Refer participants to other services and resources, including behavioral health, medical services, housing, food, clothing, and other basic needs.
• Provide in-person coverage and supports at the CCBHC walk-in center.
• Participate in developing wellness plans for self-management of chronic conditions and coordinate services to help participants achieve wellness goals.
• Assist participants in obtaining or maintaining health insurance and housing through application assistance and follow-up.
• Participate in crisis intervention, resolution, and follow-up services.
• Document services in the electronic health record (EHR) and comply with all organization, state, and federal documentation requirements.
• Facilitate active and timely communication among the CCBHC team members.
• Arrange transportation to and from medical appointments and other community integration activities as needed.

Requirements

• Successfully complete an orientation and training program in wellness case management/advocacy.
• Bachelor’s degree (B.A.) from a four-year college or university in Human Services, psychology, mental health, behavioral health, social work, human development, special education, counseling, rehabilitation, sociology, nursing, or a closely related field (preferred but not required).
• Maintain a valid state driver’s license.
• MHRTc Certified or eligible.
• Experience working in or with a primary care clinic with medical staff is strongly encouraged.
• Persons with lived experience in the Mental Health and/or Substance Abuse system are encouraged to apply.

Skills

• Knowledge in child development, family systems, special education, mental health, developmental delays, or the juvenile justice system.
• Strong organizational skills and ability to independently prioritize workload.
• Excellent interpersonal skills.
• Proficiency in electronic health records (EHR).

Pay

Hourly rate: $22.00 - $24.00, dependent upon experience.
