Join Starkey as a Targeted Case Manager and become an advocate, resource connector, and support partner for individuals with intellectual and developmental disabilities. In this role, you'll work closely with individuals, families, guardians, and service providers to coordinate services, monitor outcomes, and ensure each person receives the support they need to thrive. What You'll Do Coordinate and monitor services for individuals on your caseloadAdvocate for individuals and help them access resources and supportsBuild strong relationships with families, guardians, providers, and community partnersAssist with person-centered planning and goal achievementServe as the primary point of contact and referral source for assigned individualsEnsure services are meeting needs and supporting independence, community integration, and quality of life What We're Looking For: Required Bachelor's degree in Human Services or a related fieldOR five (5) years of related, verifiable full-time experience in developmental disability services in lieu of a degreeMinimum six (6) months of experience in developmental disability servicesValid driver's license with a good driving recordAbility to successfully complete the Targeted Case Management state test within the required timeframe Preferred One (1) year of case management experience Why Starkey?
