About the Role

This position involves providing comprehensive case management services to clients, including assessment, service planning, advocacy, and ongoing monitoring to ensure clients' needs are met.

Responsibilities

  • Conduct and document comprehensive assessments of client needs.
  • Participate in the development of client service plans consistent with assessments.
  • Arrange for delivery of needed services as identified in assessments.
  • Assist clients in accessing services through referrals or linkages.
  • Monitor client progress by tracking appointments, following up on services, and performing periodic reassessments of changing needs.
  • Perform advocacy activities on behalf of clients, such as writing letters to expedite entitlement payments or encouraging self-advocacy.
  • Prepare and maintain case records documenting contacts, services, reports, and client progress in accordance with SCS and regulatory standards.
  • Provide case consultation with service providers or collaterals to determine client status and progress.
  • Communicate with supervisors about case management issues, resourcing, service collaboration, and development of new resources.
  • Provide psychosocial education to clients and/or family members to increase understanding of illness, physical condition, or social situation.
  • Perform crisis assistance, including intervention, emergency referrals, and coordination of emergency services.
  • May serve as Primary Case Coordinator (PCC), with volume and complexity of assignments depending on skill and autonomy.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or a related field approved by Medicaid or applicable funding source.
  • Completion of state-required case management/service coordination course within six months of employment.
  • For the Adult Division: At least one (1) year of full-time employment experience post-Bachelor’s working directly with adults or ACT population (with direct daily contact) in a human service setting. Relevant Master’s degree may substitute for experience.

Physical Demands

  • Lifting of 20 pounds maximum, with frequent lifting and/or carrying of items weighing up to 10 pounds.
  • Frequent exposure to the threat of bodily harm from clients or families.
  • Exposure to blood, bodily fluids, or tissue as part of normal tasks.
  • May involve 24-hour on-call responsibilities.
  • Reliable transportation required for frequent travel.
