The Program Supervisor plays a crucial role in designing, developing, implementing, and supporting PHM initiatives. You will oversee daily operations, manage projects, and collaborate with key stakeholders to enhance patient care and outcomes.
- Analyze data to identify and address care gaps within the patient population.
- Collaborate with Practice Administrator and Leading Provider to build strong relationships with physicians, care teams, and payers.
- Oversee clinical population health strategies, annual goals, and objectives in a value-based healthcare environment.
Qualifications
- High school diploma and LVN license required.
- Experience in data collection and interpretation of data, familiarity with computer technology, and understanding of the healthcare industry.
- One year experience in implementation and evaluation of quality measures.
HOURS
8:30am-5:30pm
