Support Coordinator (Wakulla)
Overview
Provides support and resource identification functions to customers recovering from psychiatric illnesses under clinical supervision and assists with treatment. May provide substance abuse services, education, support and consultation to families. Provides crisis intervention as appropriate and serves as integral member of the crisis counseling, stabilization and therapy team.
Essential/Core Job-specific Duties And Responsibilities
- Aids clients in accessing needed financial and insurance benefits, employment, medical, social, educational, and other services.
- Provides information and referrals to assist clients with needs such as medical, dental, occupational, residential, social, and any other identified barriers to stabilization.
- Works with the client and the client's natural support system to meet the needs of clients.
- Completes the community support assessment as well as other assessment forms such as, but not limited to the Bio-Psychosocial Preliminary Assessment, Medical Questionnaire, and the Medication / Verification form.
- Develops a service plan to address the needs of the client in conjunction with the client / significant other(s).
- Maintains at least 75% (or 6 out of 8 work hours) for the provision of Case Management services in accordance with applicable clinical/quality criteria which are supported by appropriate clinical record documentation which meet Client Record Handbook specifications, SAL documentation requirements, and Agency business/clinical ethical standards.
- Coordinates care between service providers, Crisis Stabilization Unit (CSU), and other necessary services.
- Makes appropriate referrals to community resources to assist clients to meet their needs and goals.
- Documents all services provided in the client record in accordance with the Client Record Handbook Guidelines.
- Maintains at least 85% compliance with progress note requirements on Clinical Reviews conducted during rating period.
- Identifies high-risk clients and completes suicide risk assessments for all clients during the intake and/or at anytime during treatment a client are noted to have suicide ideation, plans, or an attempt.
- Reviews risk assessments with the Supervisor and /or MD/ARNP upon completion.
- Trains in SSI/SSDI/ Outreach, Access, and Recovery (SOAR) and assists eligible clients gain SSI/SSDI.
- Attends and participates in staff/unit meetings as required.
Qualifications
Requires a Bachelor's degree in Psychology, Social Work, Counseling, or related field. Requires a minimum of 2 years of experience in mental health or substance abuse field. Must have a valid driver's license and reliable transportation. Must have excellent communication skills and the ability to work independently and as part of a team. Must have knowledge of community resources and services. Must be able to pass a background check and drug screening.
