Position Overview
The Supervisor, Practice Coach provides support to Health Center Organizations (HCOs) to help them establish and reach clinical, operational, and/or quality improvement goals; adopt best practices; implement innovations; and improve the experience and delivery of care for both patients and clinicians.
Essential Duties
- Mentor and coach Practice Coaches
- Lead regular team meetings and foster a culture of collaboration, accountability, and mutual respect
- Address and resolve conflicts while maintaining team engagement and motivation
- Incentivize individual and collective performance through rewards and recognition
- Identify and implement opportunities to improve practice facilitation systems and processes for members and customers
- Prioritize, assign, and oversee tasks/deliverables across the team as appropriate
- Monitor progress to ensure timely and effective completion of tasks and deliverables
- Supervise individual performance and connect team members with appropriate resources and support to empower their success
- Provide input to leadership (Manager/Director) on staffing and resource allocation decisions
Requirements
- Master’s degree in healthcare or a related field preferred and a minimum of five (5) years of progressive experience supporting health care teams in similar or relevant role required
- Experience working in a clinical setting (safety net clinic, rural health center, county health department) and using Epic is a plus
- Demonstrated experience leading cross-functional initiatives and translating strategic priorities into measurable outcomes; previous people leadership experience is a plus
- Demonstrated strength in anticipating and resolving problems including where little to no precedent may exist, strong process understanding of project lifecycle, highly independent and self-motivated
- Demonstrated experience supporting individual clinicians, care teams, and clinic leaders with implementing practice changes in a clinical setting using principles of motivational interviewing or human centered design; practices of Model for Improvement or Lean; and frameworks such as PCMH or the Building Blocks of High Performing Primary Care
- Ability to create welcoming, structured spaces for meeting and collaboration, virtually and in-person, including creating realistic agendas and creative social connections (icebreakers), and coordinating stakeholders across disciplines and technical assistance needs
- Excellent written and verbal communication skills, with the ability to act as a coach and consultant across diverse environments and effectively engage clinicians, leadership, and staff to convey complex information and drive change and improvement
- Knowledge of change management and human-centered design required; familiarity with advanced primary care concepts, chronic disease management, and FQHC/safety net clinics preferred
- Strong facilitation skills, including the ability to design and lead effective meetings (virtual and in-person), create structured agendas, and adapt in real time and work collaboratively in team environments to maintain effective relationships with a broad range of partners
- Strong experience in a telecommuter role required, including use of electronic documentation and file management (OneNote, SharePoint), remote communication technologies (e.g., Teams, Zoom), and task management (Smartsheet); experience using JIRA is a plus
- Ability to travel up to 25% to both urban and rural locations

Work Location and Travel Requirements
OCHIN is a 100% remote organization with no physical corporate office location. Employees work remotely from home and many of our positions also support our member organizations on-site for new software installations. Nationwide travel is determined based on OCHIN business needs. Please inquire during the interview process about travel requirements for this position.

Salary Description
$74,519 - $119,231
