Supervisor, Care Progression - Social Work

The Opportunity

The Supervisor Care Progression is responsible for providing day-to-day operational oversight and leadership for Case Management and Social Work activities across inpatient units. This role supports the department’s goals of timely and effective patient discharges, coordinating with multidisciplinary teams, ensuring appropriate staffing coverage, and fostering cross-unit collaboration.

Key Responsibilities

• Oversees daily staffing assignments to ensure appropriate coverage across care progression roles.
• Remains active team member and is expected to absorb a case load as necessitated by coverage demand.
• Manages scheduling needs, including approval and tracking of PTO requests and shift adjustments.
• Counsels and instructs staff on complex discharge planning cases.
• Facilitates communication and collaboration among interdisciplinary teams to promote timely and effective discharges.
• Contributes to ongoing departmental education, including the development and delivery of training materials.
• Partners in responsibility for all resources and functions related to regulatory compliance adherence.

Qualifications

• Supervisory or lead experience strongly preferred.
• LMSW, or related licensure required, depending on background.
• Certification in Case Management (CCM, ACMA) preferred.
