About the Role

The VNS Health Behavioral Health Supervisor provides direct psychosocial services to mentally ill individuals in the community who are experiencing, or are at risk of, an acute psychological crisis or are in need of mental health treatment. Provides assessment, linkage, coordinates with, referral to and follow-up with appropriate ongoing service providers. Offers information and consultation to other community agencies and disciplines, including other services of VNS Health. Assists in the overall administrative and clinical functioning of the program.

Responsibilities

• Provides clinical supervision and direction to social work/behavioral health team members and assumes responsibility for overall program functions in the Program Manager’s absence.
• Organizes program components for appropriate staff utilization, including triage, case management, staff scheduling, referrals, outreach, evaluations, and liaison activities.
• Participates in quality assurance activities and ensures compliance with regulatory and contractual requirements.
• Screens referrals and performs psychosocial evaluations and assessments of mental health service needs and emergency social service needs for patients and their families.
• Develops and implements short-term service plans in conjunction with the team.
• Provides crisis intervention services to mentally ill individuals requiring rapid intervention.
• Offers supportive counseling, case management, and appropriate referrals for ongoing treatment.
• Prepares case histories and maintains case records in accordance with program protocols.
• Encourages service-resistant clients to accept mental health services through intervention with clients and/or concerned family members.
• Participates in interdisciplinary team meetings, rounds, and case conferences.
• Provides linkage, referral, and information to appropriate mental health and social service providers.
• Coordinates and follows up on linkages between clients and service agencies to ensure continuity of care.
• Serves as a liaison and consultant to community agencies.
• Provides outreach services to mentally ill individuals in their home or community who are at risk of an acute psychosocial crisis.
• Serves as a resource for the program and other Agency components regarding available social services and maintains a file of community referral sources.
• Participates in the development and implementation of in-service training and education.
• Assists and collaborates with the Program Coordinator/Manager in overall team functioning and assumes their responsibilities in their absence.
• Participates in community programs, education, and advocacy as requested.
• Contributes to the formulation of clinical and administrative policies and procedures and prepares policy manuals as required.
• Participates in special projects and performs other duties as assigned.

Qualifications

• Valid driver’s license required.
• Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), Licensed Mental Health Professional, or Licensed Master’s Social Worker (LMSW) in New York State required.
• National Provider Identifier (NPI) number required or must be obtained upon hire (assistance provided).
• Master’s Degree in Social Work or other Human Services-related field required.
• Minimum three years of experience as an MSW working with patients in a mental health setting required.

Pay

USD $70,200.00 - USD $87,700.00 per year.
