Substitute House Manager

About the role

Trilogy Behavioral Healthcare Inc. provides treatment services in the community, integrating various aspects of clients' lives such as mental/emotional health, physical health, employment, education, social relationships, substance use, and housing needs.

Responsibilities

• Work directly with clients regarding case management needs, training in independent living skills, and provide individual/group services or crisis intervention as needed
• Provide recovery-focused clinical services to assist with independent living tasks (e.g., personal hygiene, housekeeping, nutrition)
• Collaborate with team members, prescribers, and outreach treatment teams to deliver quality service and effective interventions
• Partner with transition teams to review and admit clients into the Residential program and oversee the safe transition into the community
• Maintain documentation and charts to ensure compliance with relevant standards and funding bodies
• Plan and facilitate special activities and outings for the clients
• Perform other related duties and/or projects as assigned

Qualifications

• Valid Illinois Driver’s License preferred
• One of the following requirements:
• A High School Diploma or GED
• An Associate's or Bachelor’s degree in any field
