Substance Use Case Manager - Barre & Hartford

About the role

Lund’s Regional Partnership Program (RPP) seeks a dedicated and driven Case Manager to support families, create meaningful change, and address substance use challenges in our communities. RPP Case Managers work out of local DCF Family Services offices to improve the well-being and permanency outcomes of children affected by parental substance misuse. This position provides support across both Barre and Hartford DCF offices, with times spent in each location varying based on referrals, caseloads and program needs.

Responsibilities

  • Work closely with DCF staff to screen for problematic substance use, link parents to appropriate treatment services, and address barriers to successful engagement.
  • Conduct substance use screenings and coordinate referrals to assessment and treatment providers.
  • Foster treatment engagement by addressing transportation, childcare, or stigma barriers.
  • Provide consultation and training to DCF staff on substance use and treatment services.
  • Liaise between the treatment system and DCF Family Services, delivering services in various settings including DCF offices, community spaces, and caregivers’ homes.

Requirements

  • A minimum of an associate degree required.
  • A bachelor's degree in social work, counseling, or a human services field is preferred.
  • Experience providing case management to persons in a substance use, recovery, or mental health treatment setting is preferred.
  • Apprentice Addiction Professional (AAP) certificate or the ability to test for certification within the first year of employment.
  • Financial support, clinical supervision, and training provided by Lund to obtain the AAP certificate.
  • Experience working with families, in multidisciplinary teams, and knowledge of community resources is preferred.
  • Ability to adhere to strong professional boundaries and maintain client confidentiality in line with state and federal regulations.
  • Skilled in the use of technology, including Microsoft Suite programs, Electronic Health Records, and video conferencing (Zoom and Teams).

Qualifications

  • Ability to maintain a valid VT Driver’s License and access to reliable transportation is required; occasional statewide travel may be required.
