About the role

Under the supervision of a Service & Support Administration Coordinator or designee, you will support individuals with developmental disabilities in determining and pursuing life goals, maintaining an individual focus, and coordinating services and supports across multiple systems. You will serve as the Board’s primary point of coordination for each person on your caseload.

Responsibilities

  • Establish an individual’s eligibility for services.
  • Provide service coordination to eligible persons.
  • Assess individual needs for services using a strengths-based approach, considering factors such as personal preferences, risks, community integration, and employment goals.
  • Develop Individual Service Plans (ISP) using Person-Centered Planning, with active participation from the individual, their selected representatives, and chosen providers.
  • Recommend plans for approval by the Ohio Department of Developmental Disabilities (DODD) when services are funded through Medicaid Home and Community Based Services (HCBS) Waivers.
  • Establish budgets for services based on assessed needs and preferred ways of meeting those needs.
  • Assist individuals in selecting providers and services through objective facilitation.
  • Ensure effective coordination of services to meet individual needs.
  • Implement a continuous quality review process of ISPs to ensure desired outcomes for the individual.
  • Take actions to remediate immediate concerns regarding individual health and safety.
  • Address results of provider compliance reviews, unusual incidents, and Major Unusual Incidents (MUIs) during team reviews to improve service quality.
  • Connect individuals to community resources to meet assessed needs and promote community life engagement.
  • Provide individuals the opportunity to designate a representative for ongoing advocacy and support related to service coordination.
  • Ensure provider compliance with licensure rules.
  • Comply with all applicable rules, regulations, laws, and Board policies and procedures.
  • Recommend policies and procedures for program operation and ensure proper implementation in accordance with state and federal requirements.
  • Assist in developing assessment tools and related surveys for service evaluation.
  • Participate in job-related training and maintain current knowledge of residential services through communication with providers.
  • Consult with providers to improve service quality, provide training and education, and participate in provider reviews.

Requirements

  • Bachelor’s Degree in Social Work from an accredited program (Master’s degree preferred).
  • OR Bachelor’s degree from an accredited college or university and one (1) year of experience in coordinating, assessing, and linking services and supports, including developing service plans for individuals. Service coordination experience with individuals with developmental disabilities or in a related field is preferred.
  • OR Bachelor’s degree from an accredited college or university and successful completion of a University or College-sponsored one (1) academic year SSA internship with LCBDD.

Credentials

  • Certification as issued by DODD.
  • Valid driver’s license.
