About the role
As an SSA, you will serve as the primary case manager for individuals with intellectual and developmental disabilities throughout Lucas County, helping them access the resources, services, and supports they need to live, work, and thrive in their communities. SSAs work alongside individuals, families, and service providers to conduct assessments, connect people with community resources, develop service plans, and authorize Medicaid Waiver services for eligible individuals.
Responsibilities
- Conduct assessments to understand individual needs and strengths
- Connect individuals with community resources and services
- Develop and implement service plans tailored to each individual's needs
- Authorize Medicaid Waiver services for eligible individuals
- Collaborate with individuals, families, and other service providers to ensure comprehensive support
Requirements
- Bachelor's degree in Social Work, Human Services, or related field
- Minimum of 2 years of relevant experience in a similar role
- Valid driver’s license and reliable transportation
- Ability to pass a background check and drug screening
Qualifications
- Strong interpersonal and communication skills
- Experience working with individuals with intellectual and developmental disabilities
- Knowledge of Medicaid Waiver programs and Ohio's Intellectual and Developmental Disabilities system
- Ability to work independently and as part of a team
Skills
- Excellent organizational and time management skills
- Proficiency in Microsoft Office Suite
- Ability to maintain confidentiality and handle sensitive information

Pay
Salary range: $35,000 - $45,000 annually
Schedule
Monday – Friday, flexible hours within a 40-hour workweek
