Student and Family Success Liaison (P/T) *Grant Funded Position Summary The Student and Family Success Liaison supports students in achieving academic, attendance, college, and career goals while strengthening family engagement and addressing barriers to student success. The coordinator provides individualized guidance, connects students and families with resources, and collaborates with school staff and community partners to support successful transitions to college, career, and postsecondary opportunities. Essential Responsibilities Build positive, supportive relationships with students and families.Monitor attendance and identify students experiencing attendance or engagement concerns.Conduct outreach to students and families to identify barriers and develop strategies for improved attendance and school engagement.Serve as a liaison to families and help promote community resources and services.Collaborate with counselors, administrators, teachers, and community partners to coordinate student support.Assist with attendance, behavior, or academic interventions as needed.Maintain accurate documentation of outreach, interventions, referrals, and student progress.Support transition, re-engagement, and problem-solving efforts for students at risk of disengagement.Promote positive family-school relationships and increase family participation in the school community.Maintain confidentiality and follow all school and district policies and procedures.Help connect community resources with Derby High School staff This position will be 5.5 hours per day, 3 days per week.
