Street Outreach Navigator

About the role

The Prince George’s County Government is seeking a Street Outreach Navigator (Community Developer 1G) within its Homeless Services Division. This position operates within an evidence-based framework, including Housing First, Trauma-Informed Care, Strengths-Based Interaction, Motivational Interviewing, and Person-Centered Communication. The role adheres to the Platinum Rule, treating all individuals the way they want to be treated.

Responsibilities

• Nestle into all parts of Prince George’s County to conduct outreach according to the Street Outreach Plan.
• Conduct Street Outreach intakes by completing necessary forms and assessment tools, if possible.
• Receive and follow up on referrals about individuals who may be unsheltered, ensuring a response within 24 hours.
• Work with clients to identify and remove barriers to emergency shelter or supportive housing, and facilitate a warm handoff as needed.
• Assist clients in identifying and accessing needed mainstream services, including navigating application processes and facilitating warm handoffs to other service providers.
• Meet with Street Outreach clients regularly (at least once per month) and complete required service information in HMIS within 24 hours of contact.
• Maintain updated client information in the Coordinated Entry Homeless Registry and collaborate with other agencies and organizations to serve clients' needs.
• Document all contacts with or about Street Outreach clients in HMIS or the Street Outreach Contact Log within 24 hours of contact.
• Collaborate and coordinate with other agencies and organizations to improve processes and procedures for better client service.
• Complete required record keeping and reporting tasks in accordance with CoC funding sources.

Qualifications

• Bachelor's Degree from an accredited college or university in Business or Public Administration, Social Service, Sociology, Psychology, or a closely related field.
• Responsible work experience, preferably including work with disadvantaged individuals.
• A valid Class C driver's license.

• Experience leading or working in a large and high-paced urban/suburban Human Services Agency.
• Lived experience.
• Excellent written and oral communication skills.
• Superior customer service.
• Ability to work well under pressure.
• Ability to excel in a team environment.
• Ability to lift 40 lbs.
• Ability to lead, inspire, motivate subordinate staff or volunteers, and delegate responsibility to them as appropriate.
• Ability to work effectively as a team member.
• Ability to work effectively with diverse populations or their group representatives.
