Mission
The Per Diem Street Medicine Physician provides reasonable and prudent medical/surgical care. They will practice appropriate management, using a harm reduction trauma informed approach with sound clinical judgment based on best practice standards and evidenced based medicine. Provides general medical care, patient education and treatment to patients of CS-HHC. Promotes and maintains health by providing services under the supervision of the Clinical Director of Homeless Care and the Chief of Medicine.
Essential Duties And Responsibilities
- Maintains confidentiality in regard to patients, staff and program matters at all times.
- Consults with other staff or specialists to make appropriate referrals for acute, chronic and preventive health care, as may be indicated.
- Collaborates with colleagues, nursing, front desk outreach, case management staff and other agencies as necessary.
- Educates patients on preventive health care and chronic disease management.
- Treats patients and staff respectfully, courteously, appropriately.
- Maintains a safe and clean working environment by complying with procedures, rules and regulations.
- Protects clients and employees by adhering to infection-control policies and protocols.
- Completes the charting of notes, forms, and coding in a timely and efficient manner. Enters appropriate legible notations, documents the encounters appropriately.
Qualifications
- Education: Degree from an accredited Medical School. Documented completion of Residency training from an accredited institution.
- CPR Certification is required.
- Experience: Board Certification or Board Eligibility. (Board certification must be attained within the timeframe dictated by one’s specialty in order to remain on staff at the CS-HHC if one was originally board-eligible at the time of one’s hire.) Connecticut Licensure. Admitting privileges at Yale-New Haven Hospital, HSR, and/or Griffin Hospital as indicated.
