Street Medicine Outreach Worker

About the Role

Access Services is hiring a full-time Street Medicine Outreach Worker to serve the community throughout Montgomery County, with an office base in Plymouth Meeting, PA. This role involves passionate, non-judgmental service to vulnerable individuals, understanding how Social Determinants of Health and the public health system impact people who are currently unhoused. The ideal candidate will engage effectively with individuals experiencing challenging life circumstances, demonstrate resilience, and work in various weather and environmental conditions.

Responsibilities

• Identify obstacles/barriers preventing individuals from utilizing available healthcare and decrease those barriers through support such as transportation to appointments and clinics.
• Partner with unhoused individuals to assess physical and behavioral health needs, including substance use disorders, using screening and assessment tools.
• Provide education on medical diagnoses, including substance use and mental health disorders, to improve health literacy and resource awareness.
• Facilitate necessary testing, evaluations, and connections to healthcare resources.
• Support Street Medicine Providers in safely navigating encampments and other locations where unhoused individuals reside.
• Assist in site-based training and education for students shadowing or interning with Access Services.
• Attend county, committee, and consumer meetings; facilitate multisystem meetings and support complex scenarios.
• Participate in regular team meetings and case reviews with Street Medicine and Street Outreach teams.
• Partner with the Montgomery County Overdose Response team and other providers to navigate treatment for Substance Use Disorder and Opioid Use Disorder.
• Provide education on HIV, Hepatitis, and TB, including referrals to testing and treatment as needed.
• Complete case notes, documentation, outcomes, statistical reports, and billing reports.
• Maintain regular attendance, work scheduled or unplanned overtime as needed, and travel during the workday using a personal or agency-owned vehicle.

Requirements

• Bachelor’s degree in public health, social work, or a related field required; Master’s degree in public health preferred.
• American Society of Addiction Medicine (ASAM) training or certification preferred.
• Experience working with individuals facing housing instability or struggling with substance use disorder and/or mental health diagnoses is desirable.

Driving Requirements

• Valid US driver’s license for at least two years (excluding permit time).
• No serious traffic convictions within the past three years (e.g., DUI, reckless driving, leaving the scene of an accident, or three or more traffic violations/at-fault accidents).
• Access to a reliable vehicle during work hours with valid registration, inspection, and auto insurance.
• Must provide proof of coverage as a driver on the insurance policy.

Skills and Abilities

• Proficiency in workplace applications, platforms, and technologies, including Microsoft Office Suite (Word, Outlook) and Evolv.
• Knowledge of homelessness and housing systems to enhance service delivery.
• Skilled in person-centered screening and assessment.
• Ability to work effectively as part of a team.

Working Conditions

• Work in program sites and the community, providing direct care and support to individuals.
• Able to stand, sit, or move quickly to ensure safety and supervision of individuals, including assisting those who have fallen, regardless of weight or size.
• Work in various weather and environmental conditions, including all outdoor elements.
• Walk, climb stairs, and travel to offsite locations as needed.
• Utilize desktop computer equipment daily in a standard office environment.
• Able to lift or move items up to 20 lbs.

Pay

$22/hour, with rates based on relevant work and educational experience.

Schedule

Monday–Friday, 40 hours weekly; 8am–4pm. This is a full-time, on-site position with no remote or hybrid options. Overtime may be required as needed.
