STNA / Home Health Aide | Weekdays - PT or FT (Home Health Agency)

Minimum of 1 year paid experience in home care required (experience with a home health agency desired). STNA or CNA preferred; HHA, PCA, DSP/DODD with 2+ years caregiving experience also considered.

Location + Travel

Bowling Green, OH and surrounding areas in Wood County. Willingness to travel to/from clients in Bowling Green and throughout Wood County is required (30-45 minute radius). For additional hours, willingness to travel up to a 45-60 minute radius from your home is required, primarily in Marion and/or Richland County.

Employment Type

Part-Time or Full-Time available with a desire for long-term commitment (2+ years). Must be willing to start immediately with part-time hours (approx. 20+ hrs/wk). A 30-day build-up to additional part-time or full-time (32-40 hrs/wk) hours is available if desired.

Schedule

Immediate availability needed; requires weekday daytime schedule (exact start and end times are customizable). Additional case hours available for full-time employment on weekday, weekend, and hybrid schedules.

Pay

Starting pay rate: $15 - $19/hr. Pay rate based on certifications, experience, schedule and travel availability, work history, references, and overall fit.

Responsibilities

  • Personal care, hygiene, and Activities of Daily Living (ADLs) including safe transfers and mobility/positioning support
  • Support safe use of assistive devices (walkers, wheelchairs, lifts) as needed
  • Nutrition & meal/meal prep support
  • Homemaking & environmental support (IADLs) including light housekeeping, errands, and tasks to maintain a clean, safe, and organized home environment
  • Community engagement & appointment support, coordination, and (in approved situations) transportation
  • Basic health and safety monitoring
  • Care plan implementation
  • Dementia & behavioral support (if applicable)
  • Demonstrated ability to follow a written Plan of Care
  • Pro-active observation and excellent electronic documentation
  • Supporting client independence, safety, and confidentiality
  • Professional, caring, and compassionate behavior to clients, client families, and peers/co-workers
  • Total care team collaboration
  • Ongoing training & certification requirements
  • Performs simple procedures as an extension of therapy services under direction and supervision of the therapist (e.g., range of motion exercises, assistance in ambulation or exercises)
  • Performs personal care activities including bathing, shampooing, skin/nail care, oral hygiene, shaving, dressing
  • Performs household services essential to healthcare at home (e.g., meal preparation/feeding, laundry, light housekeeping)
  • Assists in the patient taking medications under the direction and supervision of the Registered Nurse (opening containers or pill caddies, but not administering medications)
  • Follows infection control policies, including proper hand washing techniques
  • Reports any observed or reported changes in the client's condition and/or needs to the Registered Nurse
  • Documents care provided and completes required forms for client records
  • Promotes personal safety and a safe environment for clients
  • Demonstrates safe practice in the use of equipment
  • Communicates effectively with all members of the interdisciplinary team
  • Treats patients and families with respect and maintains confidentiality
  • Accepts and fulfills assignments with the agency
  • Participates in required in-service programs to meet compliance requirements
  • Performs other related duties and responsibilities as deemed necessary

Requirements

  • Minimum of 1 year paid in-home care or caregiving experience (agency experience desirable but not required)
  • STNAs and CNAs highly desired (or HHA/PCA/DSP with 2+ years consistent experience)
  • Exceptional on-time and attendance record, and consistent availability aligned to chosen schedule type (PT/FT/Hybrid)
  • Active CPR + FA certification and documentation of both
  • Character traits: reliable, honest, professional, compassionate, detail-oriented (in care and documentation), accountable, friendly, accommodating, respectful
  • Willingness and ability to perform required physical demands (see below)
  • Consent and ability to pass a State & Federal background check (including fingerprints)
  • 18 years or older with active Driver's License / State ID
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius

Preferred Skills & Qualifications

  • Experience with G-tubes / feeding support
  • Seizure protocols
  • Behavioral health or crisis response
  • High School diploma or GED
  • Active STNA (State Tested Nurse Aide) or CNA certification, or previous home health aide certification
  • Previous home health experience (1-3 years)
  • Familiarity with Medicaid waiver services

Main Physical Demands

  • Frequent standing, walking, stopping/bending, infrequent squatting/kneeling, and balancing
  • Weight handled: push/force required: 1-20 lbs. frequently, 20-30 lbs. occasionally, 30-50 lbs. infrequently
  • Carry/lift: 1-20 lbs. frequently, 20-30 lbs. occasionally, 30-50 lbs. infrequently
