About the Role

Home Care Network, Inc. is seeking experienced, passionate, and professional Home Health Aides (STNA & CNA highly preferred | CHHA, PCA & DSP/DODD also desired) to join our home health agency and support clients throughout Adams County, OH including Peebles, Jefferson, Rome, Manchester, West Union, Liberty, Bratton, and surrounding areas. Optional hybrid-county coverage (to secure additional hours within your travel radius) could include: Highland Co (Hillsboro), Brown County (Georgetown), Scioto County, and/or Pike County (not required).

Responsibilities

  • Personal care, hygiene, and ADL assistance
  • Light housekeeping and meal prep
  • Pro-active observation and excellent electronic documentation
  • Demonstrated ability to follow a written Plan of Care
  • Supporting client independence and safety
  • Professional, caring, and compassionate behavior to clients, client families, and your peers / co-workers

Schedule Options

  • Weekdays - during the day
  • Full-Time desired (will consider PT based on availability)
  • Local travel within Adams County | Expanded cases/hours may include adjacent Counties
  • Customizable - including Weekday, Weekend, and hybrid schedules

Requirements

  • Minimum of 1 year paid in-home care or caregiving experience
  • STNAs and CNAs highly desired (or HHA/PCA with 3+ years consistent experience)
  • Active CPR + FA certification and documentation of both
  • 18yo + active Driver's License / State ID
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid)
  • Willingness and ability to perform required physical demands
  • Consent + Ability to pass a State & Federal background check (including fingerprints)
  • Successful completion of a formal certification training program and/or a written skills test and competency evaluation
  • Meet the training and/or certification requirements of the state in which the home health aide services are being provided, including specifications of contracted agencies
  • Demonstrated ability to read, write, and follow a written Plan of Care
  • Good interpersonal skills and a passion for caring for those in need

Qualifications

  • CHHA, CNA, and STNA accreditation desirable but not required
  • Willing and able to regularly perform all physical demands of home care (including lifting/carrying up to 30–50 lbs. occasionally)
  • Willingness and aptitude to perform light housekeeping in addition to patient care services as needed
  • Current driver's license, good driving record, and/or reliable transportation is required

Essential Functions

  • Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist:
    • Range of motion exercises
    • Assistance in ambulation or exercises
  • Performs Personal Care Activities including but not limited to:
    • Bathing
    • Shampooing
    • Skin care/nail care
    • Oral hygiene
    • Shaving
    • Dressing
  • Performs household services essential to health care at home, including but not limited to:
    • Meal preparation/feeding
    • Laundry
    • Light Housekeeping
  • Assists the patient in taking medications under the direction and supervision of the Registered Nurse (opening a container or pill caddy & handing it to the patient, does not include administration)
  • Knows and follows infection control policies, including proper hand washing techniques
  • Reports any observed or reported changes in the client's condition and/or needs to the Registered Nurse
  • Documents care provided and completes the forms required for the client's records
  • Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager
  • Demonstrates safe practice in the use of equipment; does not use equipment until orientation has been provided; notifies supervisor of educational needs
  • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested
  • Treats patients and families with respect; maintains confidentiality in all aspects of the job
  • Accepts and fulfills assignments with the agency; exercises good judgment in accepting assignments
  • Participates in required in-service programs to meet compliance requirements; identifies learning needs to agency supervisor
  • Performs other related duties and responsibilities as deemed necessary

Physical Demands

  • Frequent standing, walking, stopping/bending, infrequent squatting/kneeling, and balancing
  • Weight handled - push force required: 1-20 lbs. frequently, 20-30 lbs. occasionally, 30-50 lbs. infrequently
  • Carry/lift: 1-20 lbs. frequently, 20-30 lbs. occasionally, 30-50 lbs. infrequently

Pay

  • Starting Pay Rate: $15 - $18/hr
  • Pay rate based on certifications, experience, schedule and travel availability, work history, references, and overall fit
  • Weekly pay (on Fridays) including paid onboarding and training
  • Mileage reimbursement between multiple clients is provided
  • Pay and title promotions for advanced certifications and degrees
