Staff Nurse (LPN or RN) - Residential Group Homes
Waystone is currently seeking a dedicated LPN or RN to provide medical care for individuals with intellectual and developmental disabilities in a group home setting. You will support the wellbeing of 4-5 individuals by assisting with clinical assessments, administering medications, providing treatments, and educating direct support professionals on personal care.
Schedule
- 28 hours/week (Bradford, MA): Saturday & Sunday, 7:00am - 9:00pm (benefit eligible as of date of hire)
- 8 hours/week (Haverhill, MA): Wednesday, 3:00pm - 11:00pm (not benefit eligible)
- Per diem position available (Haverhill, MA)
Pay
This is a non-exempt position.
- LPN: $32.00 - $38.30 per hour
- RN: $33.50 - $39.50 per hour

Responsibilities
- Administer medications while on-site in the assigned home
- Perform daily health assessments for all assigned individuals
- Ensure safe and effective personal care, including bathing, feeding, and mobility support
- Provide skilled nursing care as prescribed by healthcare providers
- Train and support staff in:
- Medical protocols
- Personal care techniques
- Specific treatments
- Healthy lifestyle practices
- Accompany individuals on community outings and medical appointments
- Assess medical equipment for safety, functionality, and appropriateness
- Maintain and update medical records accurately and in a timely manner
- Order medication and inventory when needed
- Review medication administration records and other medical documentation for accuracy and compliance
Requirements
- Current Massachusetts LPN or RN license in good standing
- Experience working with people with intellectual and developmental disabilities is a plus
- Ability to read, write, and comprehend English to accurately follow care plans, document services, and communicate effectively
- Organizational skills, computer literacy, and ability to document electronically
- Ability to travel within the service area of residential programs and medical appointments
- Valid driver’s license in good standing and a minimum of one year of driving experience; must be able to drive wheelchair-accessible vans
