Nurse: Residential Services (LPN or RN)

About the role

This role partners with Gracehaven therapeutic group home staff to provide the best care for each resident, while upholding state and other regulatory requirements for the home.

Responsibilities

• Provide nursing care to residents in accordance with state and regulatory requirements.
• Assist with daily living activities as needed.
• Participate in the development and implementation of individualized care plans for residents.
• Document all patient care activities accurately and timely.
• Collaborate with the therapeutic group home staff to ensure comprehensive care for residents.

Requirements

• Current license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
• Experience working with individuals who have experienced trauma.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.

Qualifications

• Bachelor's degree in Nursing preferred.
• Experience in residential care settings is a plus.
