Essential Functions, Duties and Responsibilities
- Aid in patient care coordination and management, collaborating with the patient, family, physician/nurse practitioner, and office staff.
- Perform triage in person or by phone, assisting with inquiries from clients, pharmacies, and determining acute issues.
- Implement and document laboratory services according to CLIA/CLIS regulations and procedures, including point-of-care and HIV testing.
- Act as liaison for NJIIS/VFC, ensuring appropriate vaccine entry and reconciliation by MA staff, and organizing training as necessary.
- Review and flag laboratory and diagnostic studies requiring immediate attention, coordinating with the care team/provider to inform patients.
- Provide education and counseling on various topics, including vaccine information, procedure reviews, women’s health, and primary care counseling.
- Assist in addressing community health needs, such as outbreaks, immunizations, and community education.
- Attend health fairs and other outreach events.
- Inform patients about tests and procedures, instruct them, obtain informed consent, and assist with procedures as needed.
- Offer patient education, including VFC/317 vaccinations, Title X, and Ending the Epidemic, following established guidelines and protocols.
- Manage inventories and replenish medical and clinical supplies on the Highland Health Van, including refrigerated items.
- Coordinate patient appointments and provide nurse visits, adhering to standing orders and guidelines.
- Document interventions and follow-up plans in the electronic medical record via telephone encounters.
- Support patients with medical and prescription form completion and applications for charity care, Medicaid, NJ Family Care, and other benefits or insurance.
- Advocate for patients and their families with community organizations and governmental agencies.
- Prepare reports for analysis and reporting as required by grants and center requirements.
- Report any barriers or process issues affecting optimal patient care to the QA PI Team and Director of Nursing.
- Participate in collaborative and performance improvement initiatives as directed by the Director of Nursing and Chief Medical Officer.
- Maintain patient confidentiality at all times and as per regulations and health center policies.
- Assume duties of Director of Nursing in the absence of the Director of Nursing.
Education, Training and Experience
- Successful completion of an accredited nursing program and a passing grade in the National Council Licensure Examination for Registered Nurses.
- Current and valid RN license from the state of New Jersey.
- Current BLS certification.
- Minimum of one year of experience in an ambulatory care/community health center setting.
- Experience working with community services agencies.
- Working knowledge of Zufall Health Center policies and procedures, and ability to adopt the center's mission and values.
- Efficient organizational skills.
