SSW Patient Navigator
Position Details
- Shift: 8am - 4:30pm on Tuesday's and Thursday's.
- No weekend or holiday requirement but can work on a holiday if it falls on the day of a scheduled shift, but would not be required to.
About the Role
Participate in the development and implementation of a plan of care that provides extraordinary care for the patient while being sensitive to costs and resources. Work collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties. The SSW will execute these duties under direct clinical supervision.

Requirements
- State of Utah SSW license.
- Three years of experience working with Behavioral Health Patients in a behavioral health care setting.
- Excellent interpersonal and communication skills.
- Experience adapting quickly when the need arises.
- Knowledge of available behavioral health community resources.
Qualifications
- Post high school education in a related field of study.
- Experience with discharge planning for behavioral health patients.

Pay
The hourly range for this position is $29.43 - $45.39.
Schedule
Actual weekly hours are 16.
