About the role

The Center at Waterfront is a new facility providing physical rehabilitation and complex nursing care. Our staff offers care in a hotel-like setting to help patients reach their full potential.

Primary Job Duties And Responsibilities

• Met with administration, medical and nursing staff, as well as other related departments to plan services including patients and families.
• Partnered with patients, family members, care teams, and social services staff, along with health and community agencies, to ensure smooth transitions after discharge from the Center.
• Affirmed that all documentation was informative, descriptive, and adhered to the Center's policies and procedures.
• Assisted with the admission process when necessary.
• Attended care plan meetings to offer insights and professional guidance on patients' needs.

Education/Certifications

• Bachelor's in Social Work (preferred)
• Must meet state educational requirements for a skilled nursing facility.

Qualifications

• 1 year’s experience in a skilled nursing facility and/or long-term care facility (preferred).
• Ability to make independent decisions, follow instructions, and accept constructive criticism.
