About the role
Ebenezer Square, a DePaul Community Residence-Single Room Occupancy (CR-SRO) Program, is seeking a passionate mental health professional to join our team. The SRO Residential Services Coordinator 3 (RSC 3) provides direct support, advocacy, and education to residents by assisting with the development of individualized service plans and coordinating services to enhance autonomy, quality of life, and recovery.
Responsibilities
- Provide tours and lunch visits to prospective residents and offer input to the Admission/Management team regarding acceptance into the program.
- Ensure each resident receives comprehensive, person-centered service planning, including recovery-oriented services and ongoing discharge planning, in accordance with NYS Office of Mental Health (OMH) standards.
- Conduct quarterly service plan reviews with residents and their support teams to assess progress and update plans as needed.
- Deliver rehabilitative services as outlined in OMH 595, including:
- Assertiveness/Self Advocacy Training
- Community Integration/Resource Development
- Daily Living Skills Training
- Health Services Education
- Medication Management and Training
- Parenting Training
- Rehabilitative Counseling
- Skill Development
- Socialization
- Substance Abuse Services
- Symptom Management
- Observe, monitor, document, and report resident behaviors and progress according to agency standards.
- Maintain accurate and timely resident files, including progress notes, service plans, psychiatric and medical records, and other pertinent information.
- Ensure annual appointments (physicals, eye exams, medical specialist visits, and dental appointments) are scheduled and arrange transportation as needed.
- Collaborate with Health Home Care Managers and community service providers to ensure continuity of care.
- Serve as a liaison between residents and their clinical providers to monitor mental health and well-being.
- Assist residents with daily living skills, including room upkeep, meal preparation, and laundry.
- Help residents plan for purchasing clothes and other necessities.
- Encourage residents to build healthy connections with family and communities and develop new supports and interests.
- Coordinate and participate in recreational activities for residents.
- Assist Medication Coordinators with resident medication supervision and assume dining room responsibilities during breakfast when assigned.
- Provide safe transportation for residents when necessary.
- Respect and maintain resident confidentiality and demonstrate a caring, positive attitude toward residents, staff, guests, and service providers.
- Follow all safety rules and regulations for self, residents, and staff.
- Attend staff meetings and trainings as required.
- Work as a member of the residential team, providing input for treatment/support team meetings and continued stay reviews.
- Perform any other duties necessary for the effective operation of the program and the well-being of residents.
Qualifications
Candidates must meet one of the following criteria:
- Bachelor’s degree in a Human Services field plus at least one (1) year of experience working in a Human Services field.
- Associate’s degree in a Human Services field plus at least three (3) years of related experience working in a Human Services field.
- High School diploma/GED plus at least five (5) years of experience working in a Human Services field.
Additional requirements:
- Clean/valid New York State driving record as outlined in DePaul’s personnel policy.
- Must be at least eighteen (18) years of age.
Pay
$23.75 - $24.35/hour.
Schedule
Sunday–Wednesday, 12:30 PM–11:00 PM (flexible).
