About the role
The SRO Residential Services Coordinator 3 (RSC 3) provides direct support, advocacy and education to residents in the Parkside program. Key responsibilities include developing and implementing individualized service plans, conducting quarterly service plan reviews, providing various rehabilitative services, maintaining accurate resident files, ensuring continuity of care, and supporting residents in daily living skills.
Responsibilities
- Assist residents with tours and lunch visits, and provide input to the Admission / Management team regarding resident acceptance.
- Ensure comprehensive service planning and ongoing discharge planning practices.
- Conduct quarterly service plan reviews with residents and their support teams.
- Provide teaching, monitoring, and recovery-oriented supports in accordance with OMH 595.
- Document resident behaviors and progress, and maintain accurate resident files.
- Schedule and arrange annual appointments for physicals, eye exams, medical specialist appointments, and dental appointments.
- Collaborate with the Resident’s Health Home Care Manager and other community service providers for continuity of care.
- Maintain a positive relationship with residents, families, and service providers while respecting confidentiality.
- Follow safety rules and regulations, attend staff meetings and trainings, and participate in recreational activities when needed.
- Assist with medication supervision and dining room responsibilities.
Requirements
- Bachelor’s degree in Human Services field with at least one (1) year of experience.
- Associates degree in a Human Services field with at least three (3) years of related experience.
- High School diploma/GED with at least five (5) years of experience.
- Clean/valid New York State driving record.
- At least Eighteen (18) years of age.
Qualifications
- Experience in a Human Services field.
- Knowledge of OMH 595 and its rehabilitative services.
- Ability to maintain confidentiality and provide supportive care.
- Effective communication and interpersonal skills.
Skills
- Strong organizational and time management skills.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office applications.

Pay
$23.75 - $24.35/hour
Schedule
- Sunday-Thursday: 7-3:30pm
- Tues-Sat: 8-4:30pm
- Tues-Fri: 7am-3:30pm
- Sat: 12-8:30pm
