SPOA Housing Case Manager

About the role

Short term Case Management for a caseload of residential housing Residents/Tenants needing enhanced services to support admission into CHOICES Housing Services or to transition to an alternate level of care.

Responsibilities

• Provide case management services to residents in need of enhanced support
• Support residents in transitioning to alternative levels of care
• Ensure compliance with housing and mental health service standards

Requirements

• Preferred Bachelor’s Degree in Social Work, Human Services, Community Mental Health or related field
• One year of human services, preferably Adult Mental Health
• Clean and valid NYS Drivers License
• Use of personal vehicle with mileage reimbursement
