About the role
Short term Case Management for a caseload of residential housing Residents/Tenants needing enhanced services to support admission into CHOICES Housing Services or to transition to an alternate level of care.
Responsibilities
- Provide case management services to residents in need of enhanced support
- Support residents in transitioning to alternative levels of care
- Ensure compliance with housing and mental health service standards
Requirements
- Preferred Bachelor’s Degree in Social Work, Human Services, Community Mental Health or related field
- One year of human services, preferably Adult Mental Health
- Clean and valid NYS Drivers License
- Use of personal vehicle with mileage reimbursement
