About the role
Westmoreland Community Action (WCA) is a non-profit organization dedicated to connecting individuals and families in Westmoreland County with opportunities and resources to improve their quality of life and reduce poverty. The Housing Case Manager will work within WCA’s Housing Programs, which provide interventions such as emergency supplies, transitional housing, rapid rehousing, permanent supportive housing, systems navigation, and street outreach. The role emphasizes person-centered, trauma-informed care and a commitment to diversity, equity, and inclusion, with the goal of helping participants achieve stable, permanent housing and long-term independence.
Responsibilities
- Provide case management services using a person-centered, trauma-informed approach.
- Develop and maintain rapport with participants to support goal-setting and motivation.
- Assist participants in navigating housing resources, social services, and community supports.
- Document case files accurately and collect data to track progress and outcomes.
- Collaborate with local education agencies to support school enrollment and attendance for families.
- Engage with property owners, funders, partner agencies, and external stakeholders to facilitate housing solutions.
- Meet with participants in home, community, and office settings as needed.
- Work flexible hours, including occasional weekends, evenings, and voluntary on-call schedules.
Requirements
- Experience working with individuals experiencing homelessness or housing instability.
- Experience implementing trauma-informed care and motivational interviewing techniques.
- Ability to maintain accurate case documentation and records.
- Strong organizational skills and the ability to set professional boundaries.
- Computer literacy with proficiency in Microsoft Office (Word, Excel, Outlook) and case management/CRM software.
- Effective verbal and written communication skills.
- A valid driver’s license and car insurance to meet agency requirements.
- Bachelor’s degree in sociology, social work, psychology, counseling, education, or a related field; OR two years of experience in public or private human services, including one year of mental health direct care experience.
- Act 33, Act 34, and FBI clearances.

- Proficiency in Microsoft Office (Excel, Word, PowerPoint, Teams, SharePoint, Outlook).
- Ability to work with Local Education Agencies to support school enrollment and attendance.
Physical Requirements
- Strength: Push/pull, lift/move up to 25 pounds.
- Manual dexterity: Perform moderately difficult manipulation (typing, writing, filing).
- Coordination: Foot/hand-eye coordination (driving, computer data entry).
- Mobility: Walk, stand, sit for prolonged periods, drive.
- Speech: Articulate clearly for employee/participant relations and phone contact.
- Vision: Read small print and drive a car.
- Hearing: Receive verbal information with some background noise.
- Concentration: Focus on moderate to fine detail with constant interruptions.
- Memory: Retain multiple verbal and written task assignments over extended periods.
- Environment: Work in both indoor and outdoor settings.
