Speech Therapist (ST) Home Health - Part Time

About the role

We are seeking an experienced, self-driven Part Time (Per Diem) Speech Therapist (ST) to join our Home Health team in Berkeley, CA. In this role, you will assess patients’ speech and communication needs, develop individualized treatment plans, and support their progress to enhance their quality of life. You’ll collaborate closely with our Clinical Director and Clinical Staff, working independently in the field and remotely.

Responsibilities

• Provide, coordinate, and oversee skilled speech therapy services as per the patient’s individualized plan of care, in collaboration with the Interdisciplinary Team.
• Conduct initial and ongoing evaluations of patient status.
• Develop and implement treatment plans, monitor patient progress, and report any changes in condition to the Physician, Case Manager, and/or Director of Nursing.
• Deliver educational information to patients and their families regarding speech and language disorders and treatment plans.
• Maintain accurate clinical records, submitting documentation in a timely manner per company policy.
• Adhere to all company policies and procedures to ensure the highest standards of care.

Requirements

• Current Speech Therapist (ST) License to practice in California.
• 1 year of experience in Home Health or Hospice care.
• Current CA Driver’s License.
• Able to travel.

Pay

Competitive compensation, including bonuses based on quality metrics (not measured on volume of visits). Option for a company vehicle based on availability.
