Speech Therapist (SLP) - Outpatient

About the role

We are seeking a Speech Language Pathologist to provide outpatient rehabilitation services primarily in-home for the older adult population, including private residences and assisted living facilities. The role involves assessing, diagnosing, and treating communication and swallowing disorders, developing personalized treatment plans, and coordinating care with interdisciplinary teams.

Responsibilities

• Supervising support personnel
• Documenting clinical activities
• Making referrals
• Adapting therapy programs
• Ensuring patient safety and satisfaction
• Facilitating the acquisition of communication devices
• Evaluating hearing deficits
• Instructing patients on therapeutic techniques

Qualifications

• Must hold a valid Speech Language Pathologist license in applicable states or proof of license eligibility
• Reliable transportation to patient locations
