Speech Therapist | Mobile Outpatient Therapy Provides outpatient rehabilitation services in-home for older adults, including evaluations and treatments to improve communication and swallowing functions.

Responsibilities

• Developing and implementing personalized care plans
• Supervising support personnel
• Coordinating with interdisciplinary teams
• Ensuring safety and equipment maintenance
• Conducting patient assessments
• Adapting therapy techniques
• Facilitating the use of adaptive devices
• Collaborating with patients, caregivers, and physicians to optimize outcomes
• Maintaining thorough documentation
• Referring patients for further assessments when needed

Qualifications

• Valid Speech Language Pathologist license
