Speech Therapist (SLP) - Home Health

Responsibilities

• Facilitates rehabilitation of patients with emotional, physical, or mental disabilities.
• Implements speech therapy care plans.
• Captures care with entire team.
• Edits patients and family members.
• Prepares timely, complete, and accurate clinical notes.
• Communicates effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications

• Speech language pathologist in the State of Texas.
• Home health experience highly preferred.
• Bilingual (Fluent in Spanish) encouraged to apply.
• Experience with Home Care Home Base EMR (preferred but not required).
• Knowledge of home health regulations and general healthcare compliance.
• Ability to handle physical workload.
• Good driving record.
• Proficient level experience in Microsoft Office (Word, Excel, PowerPoint, Outlook).
• Exceptional communication and people skills.
