About the Role

The Speech Therapist in Home Health assesses and evaluates patients with speech, language, swallowing, and cognitive impairments. They develop and implement personalized treatment plans, perform interventions to enhance patient well-being, and monitor progress.

Responsibilities

• Provide speech therapy services within legal scope and plan of care, collaborating with the healthcare team
• Evaluate patient function using diagnostic tests and assist in developing therapy plans
• Treat patients to improve communication and functional abilities based on their condition
• Observe, document, and report patient responses and condition changes to supervisors
• Educate patients, families, caregivers, and team members on speech therapy techniques and participation

Requirements

• Licensed in Speech Therapy in the respective state
• Current CPR certification
• Valid driver’s license, reliable transportation, or access to public transit
