About the role
An Occupational Therapist in Home Health providing assessment, evaluation, and therapy services to patients in their homes.
Responsibilities
- Perform patient assessments to determine functional status, activities of daily living, and occupational therapy needs.
- Develop and implement therapy plans in coordination with physicians and the healthcare team, utilizing evidence-based techniques within legal scope of practice.
- Observe and document patient responses to treatment, reporting changes to supervisors and physicians.
- Conduct home assessments and recommend adaptive equipment as needed.
- Provide patient-centered care, evaluate functional capabilities, and collaborate with healthcare professionals to optimize patient outcomes.
Qualifications
- Current state licensure in Occupational Therapy.
- CPR certification and a valid driver’s license with access to reliable transportation.
- Ability to work effectively in diverse home environments.
Skills
- Strong organizational, communication, and independent working skills (preferred).
