About the Role

We are seeking an Occupational Therapist to provide home health services, including patient assessments, evaluations of functional status, activities of daily living, fine motor skills, and home safety. The therapist will develop and implement individualized care plans in collaboration with physicians and the healthcare team, using evidence-based techniques.

Responsibilities

• Perform patient assessments and develop therapy plans within scope of practice and state laws
• Monitor and document patient responses and condition changes
• Coordinate with healthcare professionals to ensure effective care
• Conduct home evaluations and recommend adaptive equipment as needed

Requirements

• Current Occupational Therapy licensure and CPR certification
• Valid driver’s license, vehicle insurance, or access to transportation
• Ability to work in diverse home environments and serve clients regardless of background
