About the Role
The Speech Language Pathologist provides outpatient rehabilitation services in patients' homes or facilities, focusing on communication and swallowing disorders for older adults. The role involves evaluating, diagnosing, and developing individualized treatment plans, utilizing therapeutic techniques across cognitive-communication, speech, oral motor skills, and dysphagia.
Responsibilities
- Evaluate, diagnose, and develop treatment plans tailored to patient needs.
- Implement therapy techniques and adapt plans based on progress.
- Coordinate with team members, physicians, and caregivers for comprehensive care.
- Order supplies, maintain equipment, and ensure a safe environment.
- Supervise CFYs, students, or support staff as applicable.
- Perform routine reexaminations and modify treatment accordingly.
- Facilitate the use of adaptive communication devices and refer hearing deficits.
- Document all clinical activities accurately and timely.
Requirements
- Valid Speech Language Pathologist license in the practice state(s).
- Reliable transportation or valid driver’s license.
Skills
- Strong clinical assessment, communication, and case management skills.
- Ability to work independently and collaboratively in various settings.
