Job Summary

You will provide essential evaluation and therapeutic management for patients with speech, language, and/or cognitive disorders, improving communication, cognitive function, and quality of life. Every day you will conduct comprehensive assessments, develop individualized treatment plans, implement interventions, and monitor progress. You'll educate patients/families on strategies and goals, document encounters, and collaborate with multidisciplinary teams.

Responsibilities

• Conduct comprehensive assessments, including giving or receiving reports, obtaining patient histories, conducting physical assessments, and establishing care priorities.
• Assess patient's learning needs, readiness for discharge, and ability to manage illness after discharge.
• Complete documentation timely and submit within 24 hours of visit.
• Contact Supervisor and update on patient's progress.
• Submit schedule weekly and notify Scheduler of changes.
• Maintain productivity standards as set by the department.
• Receive and administer physician orders in a safe and timely manner.
• Develop/implement care plan and/or discharge plan for each patient, including interpreting care paths, physician orders, and progress notes.
• Provide direct patient care, as required.
• Perform Point of Care Testing where applicable.
• Contact physician with any problems or concerns, as necessary.
• Report changes in patient's condition to physician.
• Maintain a safe, clean, and therapeutic environment for patients.
• Prepare or assign all required documentation to meet legal requirements and hospital policies and procedures.
• Maintain appropriate communication with other staff members, patients, and family members, including answering questions, discussing patient care, and providing emotional support.

Required Job Requirements

• One (1) year of clinical experience with direct patient contact.
• Speech Language Pathologist license, CA, upon hire.
• Basic Life Support - CPR, upon hire.
• (Must be through the American Heart Association).
• Driver's License, CA, upon hire.
• Auto Insurance, upon hire.
