About the Role
Provide comprehensive speech therapy services in a home setting.
Responsibilities
- Evaluate patients using diagnostic assessments to determine speech, language, cognition, voice, and dysphagia needs.
- Develop and implement individualized treatment plans tailored to patients' age and capabilities.
- Establish and adjust intervention goals based on ongoing evaluations.
- Deliver direct therapy using various strategies and settings, coordinating care with the interdisciplinary team.
- Document evaluation results, treatment plans, progress notes, and reports accurately.
- Communicate effectively with patients, families, physicians, and team members regarding patient progress and needs.
- Participate in program development and supervise clinical internships for students.
Requirements
- CCC-SLP from ASHA or active clinical fellowship; or state licensure as a Speech-Language Pathologist.
- Preferred 1-3 years of experience in hospital, SNF, outpatient, or home health settings.
- Excellent communication, teamwork, and confidentiality skills.
- Ability to perform duties requiring visual acuity and effective hearing.
