H2 Health is seeking a Speech-Language Pathologist to join our outpatient team in Bluefield, Virginia. About the role

In this role, you'll work with a diverse outpatient population while providing individualized, evidence-based therapy services. You'll collaborate with Physical Therapists, Occupational Therapists, physicians, caregivers, and other healthcare professionals to help patients maximize communication, cognition, swallowing function, and independence.

Responsibilities

Perform comprehensive speech-language evaluations and develop individualized plans of care
Assess and treat communication, language, cognitive-linguistic, voice, fluency, and swallowing disorders
Deliver evidence-based interventions designed to improve communication skills, cognition, safety, and quality of life
Collaborate with patients, caregivers, physicians, and interdisciplinary team members to achieve optimal outcomes
Educate patients and families on treatment strategies and home programs
Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time
Utilize the Raintree EMR platform for documentation, scheduling, and patient management

Requirements

Master's degree in Speech-Language Pathology from an accredited program
Current Virginia Speech-Language Pathologist license or eligibility for licensure
Certificate of Clinical Competence (CCC-SLP) preferred or eligibility to complete Clinical Fellowship Year (CFY)
CPR certification or willingness to obtain as required
Strong communication and interpersonal skills
Commitment to providing high-quality, patient-centered care
