Speech Language Pathologist (SLP)

About the role
As a Home Health Speech Language Pathologist (SLP) for Interim HealthCare, you’ll provide home-based speech therapy to patients with communication, swallowing, or cognitive difficulties caused by illness or injury. You’ll work within a multidisciplinary home health team—including RNs, LPNs, CNAs, HHAs, PTs, and OTs—to deliver personalized care aligned with each patient’s plan and goals. Responsibilities
- Provide home-based speech therapy to patients experiencing communication, swallowing, or cognitive deficits due to illness or injury.
- Collaborate with a home health team (RN, LPN, CNA, HHA, PT, OT) to implement and adjust patient care plans and goals.
- Observe, evaluate, and document patient progress in communication, swallowing, and cognitive processing.
- Conduct comprehensive patient assessments and ensure therapy goals are met.
- Guide patients through exercises targeting mouth, breathing, swallowing, fluency, articulation, and reading.
- Educate patients and families on care plans, therapy goals, safety practices, and self-care techniques.
Requirements
- Master’s or Doctoral degree in Speech Language Pathology.
- Active Colorado SLP license.
- Minimum of one year of speech therapy experience, preferably in home healthcare.
- Current CPR certification (demonstration course required).
- Pass a background check and drug test (THC screening included).
- Knowledge of state and federal home health regulations.
- Strong clinical judgment, critical thinking, communication skills, empathy, and compassion.
Pay
Visit rates: $75 – $125 per visit, with competitive weekly pay including travel time and mileage reimbursement.
