Territory

Cleveland, Garfield Heights, Solon, Brecksville, Broadview Heights, Twinsburg, Northfield, Euclid, Highland Heights, Beachwood, Willowick, Willoughby, Mentor, Chesterland, Gates Mills, Mentor, and surrounding areas.

Schedule

3 weekdays; 8:30 am to 5:00 pm

Pay

New guaranteed base hourly rate plus a generous uncapped bonus structure. Mileage is paid, and additional bonus opportunities are available. Sign On Bonus: $2,500.00.

Responsibilities

  • Provide home-based skilled speech therapy care for patients as directed by the attending physician
  • Educate patients and their family members and ensure patient safety
  • Develop an individualized speech therapy plan of care and adjust as needs change
  • Guide the patient in therapeutic, creative, and self-care activities to improve functioning
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and agency staff
  • Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, communication, and related functions
  • Help decrease re-hospitalizations by teaching preventative measures and decision-making
  • Counsel patients and families on managing the disease/injury/disability in all phases of speech therapy
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines
  • Understand and follow agency policies, procedures, rules, and regulations
  • Communicate changes in schedule/availability to schedulers or supervisors
  • Attend in-service training and mandatory agency meetings

Requirements

  • Current Speech Therapist license in the state of employment
  • Minimum one-year experience as a Speech Therapist in an acute care setting
  • Home care experience preferred
  • Valid driver’s license and auto insurance in your name as a driver
  • Capable of meeting all physical demands of the role
