About the role

Responsibilities
- Conduct evaluations
- Develop and implement evidence-based treatment plans
- Support patient mobility and independence
- Document in EMR
- Collaborate with interdisciplinary teams
- Educate patients and caregivers
- Utilize telehealth when appropriate
Qualifications
- Valid state license or eligibility
- Degree from an accredited program
- Basic computer and EMR proficiency
- Current CPR certification
- Ability to meet physical demands such as lifting, repositioning, and driving
