Speech Language Pathologist

About the role

Seeking licensed Speech-Language Pathologists to provide one-on-one care to older adults in private residences and senior living communities within a community-based, flexible model. The role emphasizes delivering evidence-based assessments, developing individualized treatment plans, and supporting patient wellness, mobility, and independence. Clinicians may incorporate telehealth and virtual care to enhance service delivery.

Responsibilities

• Perform evaluations, create treatment plans, and deliver therapy to promote functional independence.
• Document patient progress accurately within electronic medical records.
• Collaborate with interdisciplinary teams including PTs, OTs, and fitness specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Utilize telehealth/virtual care to extend reach and flexibility.

Qualifications

• Valid state license or eligibility, with a degree from an accredited program.
• Current CPR certification and ASHA CCC credential.
• Proficiency in basic computer and EMR systems.
• Ability to meet physical demands, including lifting, repositioning patients, and driving.
