Speech Language Pathologist

About the role

Provide home-based evaluation and treatment for adult patients.

Responsibilities

• Assess speech, language, cognitive-communication, and swallowing needs.
• Develop evidence-based care plans tailored to individual patients.
• Educate patients and caregivers in the home setting.
• Collaborate with an interdisciplinary team and document in the EMR.
• Participate in case conferences and support quality improvement initiatives.
