Seeking a licensed Speech Language Pathologist to provide personalized, in-home and community-based care to older adults, promoting mobility, independence, and quality of life. Key Responsibilities Include Conducting evaluations, developing individualized treatment plans, and delivering evidence-based therapy.Supporting patient wellness, functional independence, and mobility through therapy sessions.Documenting care accurately using electronic medical records in compliance with regulations.Collaborating with interdisciplinary teams including PTs, OTs, and caregivers.Educating patients, families, and staff to enhance safety and aging-in-place outcomes.Utilizing telehealth and virtual care options to increase flexibility and access. Qualifications And Skills Include Valid state licensure or eligibility, with a degree from an accredited Speech Language Pathology program.Proficiency in basic computer skills and EMR systems.Current CPR certification and ASHA CCC.Ability to meet physical demands such as lifting, repositioning patients, and driving between locations.
