Responsibilities
- Provide one-on-one speech therapy to older adults in private homes and senior living communities.
- Conduct evaluations, develop personalized care plans, and deliver evidence-based treatments.
- Support patient wellness, mobility, independence, and aging-in-place goals.
- Document therapy activities accurately within the EMR system, complying with regulations.
- Collaborate with interdisciplinary teams including PTs, OTs, and fitness specialists.
- Educate patients, caregivers, and staff on safety and functional aging strategies.
- Utilize telehealth and virtual care to enhance service flexibility and reach.
Qualifications & Skills
- Valid state licensure or eligibility for Speech Language Pathology.
- Degree from accredited SLP program and current CCC from ASHA.
- Proficiency with computers and EMR systems.
- Current CPR certification.
- Ability to meet physical demands, including lifting, repositioning patients, and driving.
