Responsibilities

  • Provide one-on-one speech therapy to older adults in private homes and senior living communities.
  • Conduct evaluations, develop personalized care plans, and deliver evidence-based treatments.
  • Support patient wellness, mobility, independence, and aging-in-place goals.
  • Document therapy activities accurately within the EMR system, complying with regulations.
  • Collaborate with interdisciplinary teams including PTs, OTs, and fitness specialists.
  • Educate patients, caregivers, and staff on safety and functional aging strategies.
  • Utilize telehealth and virtual care to enhance service flexibility and reach.

Qualifications & Skills

  • Valid state licensure or eligibility for Speech Language Pathology.
  • Degree from accredited SLP program and current CCC from ASHA.
  • Proficiency with computers and EMR systems.
  • Current CPR certification.
  • Ability to meet physical demands, including lifting, repositioning patients, and driving.
