About the Role
The Speech Language Pathologist (SLP) provides personalized, one-on-one speech therapy to older adults in private homes and senior living communities, supporting their independence, mobility, and quality of life through evidence-based treatment plans.
Responsibilities
- Perform evaluations, develop individualized care plans, and deliver therapy sessions.
- Document patient progress accurately within the EMR system.
- Collaborate with an interdisciplinary team including PTs, OTs, and fitness specialists.
- Educate patients, caregivers, and staff on safety and aging-in-place strategies.
- Utilize telehealth and virtual care to extend reach and flexibility.
Requirements
- Valid state licensure or eligibility as a Speech Language Pathologist.
- Degree from an accredited SLP program and current ASHA CCC certification.
- Proficiency in basic computer and EMR use.
- Current CPR certification.
Physical & Other Requirements
Ability to meet physical demands of community-based care, including lifting, repositioning patients, driving, and wearing PPE.
